Military Move Claims

Please take a moment to look over the Tips and Instructions below and then submit your Claim Form.
Tips and Instructions for filing a claim with Cartwright:
- If your shipment moved through DPS, the military requires you to file your claim through the DPS website: https://eta.sddc.army.mil/ETASSOPortal/default.aspx
- You can identify whether or not your shipment is a DPS shipment by the shipment reference number (GBL Number) DPS shipments have GBL numbers with 4 letters and 7 numbers, for example ABCD1234567, whereas other shipments have two letters and 6 numbers AB123456.
- Please allow us 60 days to settle your claim from the
time we receive a completed Claim Form. A Claim Form is complete only
after Cartwright has received all required documentation.
- Failure to submit a completed Claim Form will result in your claim being delayed.
- Cartwright reserves the right to inspect all damage, therefore,
you must retain all items you are claiming until Cartwright or Transpo
Service Limited releases them for disposal. Failure to do so may result
in denial of your claim.
- In many instances pictures and original purchase receipts are
required. Once an adjudicator reviews your claim they will advise you
what is needed.
- Full Replacement Value is only paid for items that are completely destroyed or missing.
- Fill out Sections 1; 5-10; 11a or 11b; 12-13; and 23 carefully
before submitting your form. If any of these sections are not filled out
your form will be returned to you.
- Separate your shipments as you record damages (i.e. keep your
household goods and baggage separate). Cartwright can only settle for
damages on shipments that were tendered to us and we can only settle on
one type of shipment at a time.
- Pictures and supporting documents are necessary for all items valued at more than $100.00.
- You must include the following with your Claim: Current mailing
address; Your GBL Number; Repair estimates; Insurance documentation
verifying value; Receipts; DD Form 1840 and DD Form 1840R.

- To file your claim online with us, please register below. You will be directed to the proper online form to complete. When using the online form, you can also upload pictures for the items you are claiming. This helps us determine whether repairs are viable and to propose replacement with similar items. For electronic items, please include a picture of the manufacturers label showing model and serial number information.
